• By participating in the Event, you confirmed that you have read, understood and agree with the following terms and conditions.
• HomeTeamNS is not liable for any damages or injuries caused or incurred by the participant.
• HomeTeamNS reserves the right without refund or compensation to refuse admission/evict any person(s) whose conduct is disorderly or  inappropriate or who poses a threat to security, or to the enjoyment of the services by others.
• By participating, participants agree to be photographed and video recorded for the purpose of publicity and for use in future publicity collaterals.
• By participating, participants understand and agree that HomeTeamNS may collect, use and disclose your personal data, as provided in this form for the sole purpose of administering this activity. We assure you that proper safeguarding measures are in place to protect your personal data in accordance to PDPA guidelines.
• HomeTeamNS reserves the right to cancel or postpone the event.
• HomeTeamNS shall not be held responsible for any claims arising out of any death or injury, damage or loss, suffered or caused during participation, and this includes all costs and expenses incurred as a result of such claims.
• HomeTeamNS management reserves the right to amend the terms & conditions as necessary.

• I declare that all particulars above are true and correct.
• I agree to abide by the aforementioned Terms & Conditions.
• I am aware that the registered participants will undergo the event at my own risk and thereby release HomeTeamNS, its staff, and other organizing persons from liabilities, which may arise during the duration of the event.
• I agree that fees are non-refundable unless the activity is cancelled by HomeTeamNS.
• I acknowledge that Parent / Guardian consent is required for applicants under 21 years old.
• I hereby declare the participant to be in good physical and mental health to take part in this event.

HomeTeamNS recognizes its obligation under the Personal Data Protection Act 2012 (PDPA) which include the collection, use and disclose of personal data (information provided above) which you have given consent to by the submission of this registration form.

Your personal data belonging to you and your family may be used and disclosed to HomeTeamNS’ affiliated organisations, third party service providers (activity partners etc.) for the following purposes:

1. Register and contact you about your attendance at the activity;
2. Carry out administrative, planning activities and to facilitate your requirements for the activity;
3. Plan and develop future activities and opportunities;
4. Compilation and analysis of statistics relevant to the course/activity;
5. Providing marketing material that you have agreed to receive.

As a valued HomeTeamNS member/customer, you can expect HomeTeamNS and its affiliated organizations, service providers and programme partners to communicate to you via channels such as electronic direct mailers (Email), Short messaging service (SMS), direct mailers (Postal Mail), Whatsapp and phone calls (Voice).

By attending the activity stated above, you consent to the use of photograph, voice, comments and image in any broadcast of this event and in subsequent productions drawn from video or audio recordings of this event. The photographs and recordings may be published or broadcasted in the official HomeTeamNS and HomeTeamNS affiliates’ publications and in publicity materials, including the HomeTeamNS and HomeTeamNS affiliates’ websites and social media. All recordings, articles, quotes, photographs, films, CCTV footage, audio or video and/or any reproductions of same in any form, are the property of HomeTeamNS. HomeTeamNS and its affiliates, employees, representatives and agents are released from any and all claims, demands, costs and liability that may arise from the use of any of the described above, arising out of being interviewed, recorded, photograph, videotaped or filmed.

**It is the responsibility of event participants to remove themselves from group and candid shots/recordings that the photographers/videographers intend to capture, if they do not wish to have their photos/audio-visual recordings taken at the event.**
